Indications and usage:
Accutane (Isotretinoin) is a retinoid indicated for the treatment of severe recalcitrant nodular acne in patients 12 years of age and older.
Limitations of use
In the USA isotretinoin may only be administered to patients enrolled in the iPLEDGE program (a risk management distribution program mandated by the U.S. FDA).
Dosage and administration:
Recommended dosage of isotretinoin is 0.5 to 1 mg/kg/day given in two divided doses without regards to meals for 15 to 20 weeks.
Once daily dosing is not recommended.
Perform pregnancy tests prior to prescribing, each month during therapy, end of therapy, and one month after discontinuation.
Prior to prescribing, perform fasting lipid profile and liver function tests.
Accutane is not substitutable with other forms of isotretinoin.

Overdosage:

Warnings and precautions:

Lipid abnormalities: triglyceridemia, low HDL and elevation of cholesterol. Monitor lipid levels at regular intervals.
Hearing impairment.
Hepatotoxicity: monitor liver function tests at regular intervals.
Inflammatory bowel disease.
Skeletal abnormalities: back pain, arthralgias, decreases in bone mineral density and premature epiphyseal closure.
Ocular abnormalities: corneal opacities, decreased night vision.
Glucose and CPK abnormalities.
Side effects, adverse reactions:
Most common adverse reactions of isotretinoin pills (incidence >= 5%) are: dry skin, lip dry, back pain, dry eye, epistaxis, arthralgia, headache, nasopharyngitis, dermatitis, chapped lips, blood creatine kinase increased, musculoskeletal discomfort, chelitis, upper respiratory tract infection, visual acuity reduced.
