Isotretinoin (trade names include Accutane, Aknenormin, Amnesteem, Claravis, Curacne, Isotretinoin Mepha, Isotretinoin Ratiopharm, Myorisan, Oratane, Roaccutan, Roaccutane, Sotret, Tretinac, Tufacne) is an anti-acne medication from retinoids (Vitamin A derivatives) for topical and systemic use pharmacological groups. This medicine is used for the treatment of moderate to severe acne, for nearly all types of breakouts, acne present for many years, acne that has not responded completely to antibiotic pills, anti-acne creams, gels and lotions. Isotretinoin is also indicated to prevent certain skin cancers (such as squamous-cell carcinoma), neuroblastoma, acute nonlymphocytic leukemia, granuloma annulare, metastatic melanoma, some other cancers, harlequin-type ichthyosis, lamellar ichthyosis, hidradenitis suppurativa, severe rosacea, xeroderma pigmentosum, keratoses, pityriasis rubra pilaris, genital warts, recalcitrant condylomata acuminata of the cervix, fibrodysplasia ossificans progressiva. It works by reducing skin oil production, changing the characteristics of the skin oil, and preventing abnormal hardening of the skin.

Indications and usage:
Accutane (Isotretinoin) is a retinoid indicated for the treatment of severe recalcitrant nodular acne in patients 12 years of age and older.
Limitations of use
In the USA isotretinoin may only be administered to patients enrolled in the iPLEDGE program (a risk management distribution program mandated by the U.S. FDA).
Dosage and administration:
Recommended dosage of isotretinoin is 0.5 to 1 mg/kg/day given in two divided doses without regards to meals for 15 to 20 weeks.
Once daily dosing is not recommended.
Perform pregnancy tests prior to prescribing, each month during therapy, end of therapy, and one month after discontinuation.
Prior to prescribing, perform fasting lipid profile and liver function tests.
Accutane is not substitutable with other forms of isotretinoin.
Dosage forms and strengths:
Accutane (Isotretinoin) capsules: 10 mg, 20 mg, 30 mg and 40 mg.
Overdosage:
In humans, overdosage by isotretinoin has been associated with vomiting, facial flushing, cheilosis, abdominal pain, headache, dizziness, and ataxia. These symptoms quickly resolve without apparent residual effects.
Contraindications:
Pregnancy.
Hypersensitivity to Isotretinoin (Accutane) capsules or any of its components.
Warnings and precautions:
Unacceptable contraception: micro-dosed progesterone preparations are not an acceptable method of contraception during therapy with Accutane (Isotretinoin) capsules.
Psychiatric disorders: psychosis, depression, suicidal thoughts and behavior, and aggressive and/or violent behaviors.
Pseudotumor cerebri, some cases with concomitant tetracyclines.
Serious skin reactions: Stevens-Johnson syndrome, toxic epidermal necrolysis.
Acute pancreatitis, rarely fatal hemorrhagic pancreatitis, in patients with either elevated or normal serum triglyceride levels.
Lipid abnormalities: triglyceridemia, low HDL and elevation of cholesterol. Monitor lipid levels at regular intervals.
Hearing impairment.
Hepatotoxicity: monitor liver function tests at regular intervals.
Inflammatory bowel disease.
Skeletal abnormalities: back pain, arthralgias, decreases in bone mineral density and premature epiphyseal closure.
Ocular abnormalities: corneal opacities, decreased night vision.
Glucose and CPK abnormalities.
Side effects, adverse reactions:
Most common adverse reactions of Accutane (Isotretinoin) capsules (incidence >= 5%) are: dry skin, lip dry, back pain, dry eye, epistaxis, arthralgia, headache, nasopharyngitis, dermatitis, chapped lips, blood creatine kinase increased, musculoskeletal discomfort, chelitis, upper respiratory tract infection, visual acuity reduced. To report suspected adverse reactions, contact Roche, Genentech, FDA or iPLEDGE.
Drug interactions:
Vitamin A: may cause additive adverse reactions.
Tetracyclines: avoid concomitant use.
St. John’s Wort: may interfere with oral contraceptives.
